
MEDICATION REVIEW AT YOUR GP PRACTICE
A medication review is important to check that you are receiving the treatment you need for your clinical conditions and that the medicines you take work well together.
This page tells you what to expect at your medication review, and lets you record any notes you might want to discuss at the review on your phone or device (a printable version is also available here).
Who is involved in the medication review?
- You are welcome to bring a family member or friend to the review.
- Medication review usually involves an appointment with a Clinical Pharmacist or a GP at your practice.
- Clinical Pharmacists are highly qualified experts in medicines and managing long-term conditions.

What happens during the review?
- Each medicine you take for your medical conditions will be looked at and discussed.
- You can talk about any concerns you have, ask questions, and seek information and advice.
- Changes to your medicines may be suggested by you or your reviewer.
- You can be involved in discussion and decision making around the medicines that you use.
- You may find it helpful to bring your medicines with you to the appointment, although this is not essential.
How you can prepare for the medication review
It can be helpful to think about each of the medicines you take, and what they are for. The kind of things your reviewer may want to talk to you about include:
- If you know why you are taking each medicine.
- Any non-prescription medicines you use.
- New symptoms or possible side effects.
- How and when you are using each medicine.
- Problems you have taking any of your medicines.
- Possible changes which may be helpful.
